❤️ Not Every High Troponin Is a Heart Attack: Understanding the 3 Types of MI
Abstract

A high troponin level means there is injury to the heart muscle but it does not automatically mean a heart attack. The key is determining whether the injury is caused by acute ischemia and understanding the underlying mechanism. Heart attacks are now broadly categorized into three clinical types: primary MI, secondary MI, and procedure-related MI. Recognizing the difference matters because the cause of the heart injury can determine the treatment.
❤️ Did you know there’s more than ONE way to have a heart attack?
As a cardiologist, I used to get this question all the time:
“Did I really have a heart attack or not?” 🤔
Most people think:
🫀 “A heart artery gets blocked = heart attack.”
But it’s more complicated and not always easy to explain in simple words.
🧪 There’s a blood test called troponin that helps us detect injury to the heart muscle.
But here’s the important part:
⚠️ A high troponin does NOT automatically mean a heart attack.
Troponin tells us there is heart muscle injury. Doctors then need to determine WHY that injury occurred.
❤️🩹 Heart attacks are now broadly categorized into 3 clinical types: (previously classified into 5 types)
🔴 Primary MI - a problem in a coronary artery, often involving plaque disruption and a blood clot, reduces blood flow to the heart muscle.
🟠 Secondary MI - another serious problem creates an imbalance between the heart’s oxygen supply and demand. This can happen with severe anemia, infection or other critical illness, a blood clot in the lungs, very low blood pressure, very high blood pressure, a very fast heart rhythm, and other conditions.
🔵 Procedure-related MI - a heart attack that occurs in connection with certain heart procedures.
🚨 And here’s the BIG takeaway:
High troponin ≠ automatically a heart attack.
🧪 Troponin = heart muscle injury.
Doctors need to determine whether that injury is actually due to a heart attack and, if so, what caused it.
