🧬 GLP-1 Series: Lilly’s TRANSCEND-T2D-2 topline results
I’ve previously described Eli Lilly as the NVIDIA of healthcare.
Its momentum in China’s GLP-1 market is worth watching. In Q2 2026, Lilly led weekly GLP-1 weight-loss drug sales on major Chinese e-commerce platforms.
Today, Lilly announced the topline results from TRANSCEND-T2D-2.
Now, let’s talk about retatrutide.
At 80 weeks, retatrutide delivered more than double the weight loss of Ozempic 2 mg:
• Weight loss: 20.9% vs 9.4%
• A1c reduction: 2.2 vs 1.8 percentage points
Why is retatrutide different?
It’s a triple-hormone receptor agonist targeting:
• GLP-1: appetite regulation and glucose control
• GIP: glucose-dependent insulin secretion and metabolic regulation
• Glucagon: may increase energy expenditure
Unlike semaglutide (GLP-1) or tirzepatide (GLP-1 + GIP), retatrutide activates all three pathways.
The trade-off? Gastrointestinal side effects and treatment discontinuations were more frequent at the higher dose.
Why does this matter? Obesity is linked to multiple cardiometabolic conditions, including type 2 diabetes, hypertension, dyslipidemia, heart attack, OSA, PAD, MASLD and HFpEF.
The big question: Will greater weight loss translate into fewer heart attacks, strokes, and cardiovascular deaths?
Retatrutide remains investigational and is not FDA-approved.
